In one in four cases, the cause of anosmia can not be identified. We present a patient with a large olfactory meningioma as the cause of idiopathic anosmia. The patient had no other symptoms of intracranial tumor at all. The medical history emphasizes the importance of MRI scan of the brain in the examination of possible idiopathic anosmia.
