      <Abstract>NSAID related spontaneous rectal perforation is a rare and severe condition with potentially life-threatening outcomes. We report a case of a 75-year-old woman with a mid-rectal perforation related to prolonged NSAID use. She presented with sudden rectal and lower abdominal pain. Imaging revealed a localised gas collection in the pre-sacral space without widespread peritonitis. Management included laparoscopic loop sigmoid colostomy and IV antibiotics. The patient recovered well and showed complete resolution at follow-up. This case highlights the importance of judicious use of NSAIDS in high-risk patients and underscores the need for individualised management strategies.</Abstract>
